General Description of the Job Class Provide pharmaceutical car eservices to the patients of the DukeUniversity Health Systemincluding, butnot limited to oversight, monitoring, preparation,dispensingand prope rdocumentation of patient medication therapy as well as providing drugin formation, patientinformation and oversight of information and dispensin gsystems; ensurecompliance with Federal,State and local laws and regulat ions as well as proper storage anddocumentation. May alsoadvise physicia nson matters pertaining to drug usage and control;participate in the dev elopmentand delivery of continuing education programs for Pharmacy, Pati entCare Services, Medical Staffand students. Duties and Responsibiliti es of this Level Review andmonitor patient's medication therapy and ord ers written byappropriate caregivers forsafety and efficacy considerings pecific patient conditions and healthstatus; advise patient careteam ofi ssues with, orpossible enhancements to, patient's medicationtherapy andd ocumentsuch interventions. Prepare, label and dispense medications forpa tients in accordance withphysicians' prescriptions; ensure compliancewit h Federal, State andlocal laws and regulationspertaining to the dispensi ng of drugs and narcotics and themaintenanceof required records. Mainta in appropriate records of dispensing and oversee and maintain thesystems that charge orcredit for medications and medication treatments. Provide drug information and educate caregivers, patients and studentsregarding medications andtheir proper use and monitoring. Oversee and monitorall aspects of themedication process;collect data and present reports andrec ommendations regarding issueswith the medicationsystems. Ensurethatapp ropriate medications are available for patients andproperly procured, pr epared,stored andmaintained. Supervise the process and technical personn el who assist inpreparation, maintenance anddispensing of drugsandsuppli es. Provide support, education and oversight for investigationalmedications for patients; provideinformation regarding their preparation,labelin g and availabilitywithin the medication system;maintain appropriate and required records and documentation forinvestigational drugs.Provide spec ialized pharmaceutical services to improve drug usage andtherapeutic out comesincluding advising physicians on issues concerning drug therapy, th einherent toxicity of drugs andside effects, as well as assistingin the prescription of appropriatedoses. Contributes to the education of studen ts and pharmacy residents. Maintain liaison relationshipswith medical an d nursing staff; providetimely information pertainingto pharmaceutical s upplies, drug usage and compatibility, stateandfederal regulations regar dingdrug controls, and Joint Commission standards. Conduct and evaluate medication histories, assess compliance andsuggest modificationsso as to achieve desired outcomes; instruct patients in the proper use ofprescrib ed drugs; participate inpatient care rounds with multidisciplinary teams to evaluate patientprogress. Individualize medicationregimensusing soun d principles, accountingfor pharmacodynamic andpharmacokineticvariations in drug absorption, distribution, metabolismand eliminationwithresponsi bility for establishing and continually improving the deliveryof pharmac eutical care topatients withinareas of direct responsibilityand assistin g others inthe department with the same. Conduct and participate in rese arch including, but not limited to,performing as the primaryinvestigator orco-investigator for research programs which will impacton thedeliver y of qualitycare or examine the pharmacoeconomicimpact of providing care to thepatients in the DukeHealth System. Perform other related duties i ncidental to the work described herein. Required Qualifications at this Level Education:Doctor of Pharmacy DegreeOR BS Pharmacy Degree Experience:With a BS Pharmacy degree, two years of pharmacy practice e xperience,oracceptable boardcertification, or completion ofan accredited residency program anddemonstrated abilityto achievepositive patient out comes in a similar environment is required. With Doctor ofPharmacy degre e, no experience is required. Degrees, Licensure, and/or Certificat ion:Licensed Pharmacist or eligible to practicepharmacyin North Carolin a Knowledge, Skills, and Abilities:Ability to communicate clearly in oral and written form. Extensive knowledge of drug products,medical ther apeutics,pharmacokinetics, and diseasestates. Ability towork with inform ation systems, automated andtechnical equipment.
